What would you think she meant?

I would think the dang toothpick stuck her in the thigh, leg, foot, somehow in her seat or something and she looked away from the road to snatch it from imbedding in her flesh as she drove. But while looking at it and asking herself, 'Why was the toothpick there, why there?' BOOM, accident. Of course the question will then remain 'Why WAS the toothpick there, why?"
