What form in Riverside County, California do I file for a "Petition for Writ of Administrative Mandate"?

Im trying to appeal a decision from the DMV revoking my ambulance driver license. Im told to file a petition for a writ of administrative mandate, however I cannot locate this form anywhere. Thanks for the help.
